Connecting a Year-End Personal Impact Plan Questionnaire


1. Reflecting on 2024 Achievements and Lessons Learned

  • What were your top three accomplishments this year? Describe the impact of each.

  • What key lessons did you learn from challenges or setbacks in 2024?

  • Were there specific moments this year that inspired you or shifted your perspective? What were they, and why were they impactful?

  • What feedback did you receive from colleagues, mentors, or team members? How did this feedback influence you?


2. Evaluating Personal and Professional Growth

  • In what ways did you grow as a leader this year? Consider both skills and mindset shifts.

  • How did you manage your work-life balance, and what could be improved?

  • Did you acquire new skills or knowledge? How did these contribute to your professional effectiveness?

  • Reflecting on your energy levels, which activities or projects invigorated you? Which drained you?


3. Identifying Your Core Values and Vision for 2025

  • What are the top three values that guide your decisions and actions as a professional?

  • How well did your work this year align with these values?

  • What vision do you have for your role and impact in 2025? Think about both professional goals and personal aspirations.

  • How would you define success at the end of 2025?


4. Setting Priorities for Maximum Impact

  • What are the top three areas in your work where you want to make a greater impact next year?

  • Which ongoing or new projects align best with your vision and values for 2025?

  • Are there specific skills, tools, or resources you need to enhance your effectiveness?

  • What actions will you take to improve team alignment and engagement next year?


5. Creating an Action Plan with Measurable Goals

  • Based on your priorities, what are three specific, measurable goals you want to achieve in 2025?

  • What are the key milestones you’ll need to hit to stay on track for each goal?

  • Who will you need to involve or collaborate with to accomplish these goals?

  • How will you track your progress and hold yourself accountable throughout the year? Perhaps invest in a coach who will help to guide you along the way.


6. Planning for Personal Well-being and Balance

  • What personal commitments or routines (e.g., health, hobbies, family time) are important for your well-being?

  • How will you ensure these commitments are honoured alongside your professional goals?

  • Is there a particular stress-management technique or self-care practice you’d like to prioritise in 2025?

  • What boundaries will you set to safeguard your time and energy?


7. Expanding Your Network and Influence

  • Who are three people you’d like to connect or reconnect with next year? Why?

  • How do you plan to expand or deepen your professional network?

  • Are there mentorship or community-building opportunities you’d like to explore?

  • How will you actively seek and provide feedback to grow relationships within and outside your organisation?


8. Looking Beyond 2025: Long-term Goals and Legacy

  • What long-term impact do you hope to achieve in your career and personal life?

  • In what ways can you start building toward that legacy in 2025?

  • How do you want to be remembered as a leader, colleague, or mentor?

  • What additional resources or support might you need to pursue your broader goals?


By reflecting on these questions - and your honest answers - you can gain a clearer view of your achievements, set aligned goals for the upcoming year, and develop a personal impact plan that is actionable, sustainable, and personally meaningful.
